Our Commitment to Your Privacy

Dr. Rebecca Sinai, DO (“the Practice”) is committed to protecting the privacy of your health information. This Notice of Privacy Practices (“Notice”) describes how we may use and disclose your protected health information (PHI) to carry out treatment, payment, or health care operations and for other purposes permitted or required by law. It also describes your rights regarding your health information and how you can exercise those rights.

We are required by the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and its implementing regulations to maintain the privacy of your PHI, provide you with this Notice of our legal duties and privacy practices with respect to your PHI, and abide by the terms of the Notice currently in effect.

How We May Use and Disclose Your Health Information

Treatment

We may use your PHI to provide, coordinate, or manage your health care and related services. This includes sharing your health information with other health care providers involved in your care, such as specialists, pharmacies, laboratories, and hospitals.

Payment

We may use and disclose your PHI to obtain payment for services provided to you. This may include submitting claims to your health insurance plan, verifying coverage, and collecting amounts owed.

Health Care Operations

We may use and disclose your PHI for our health care operations, which include quality assessment, employee review, training, accreditation, and other activities necessary to run our practice and ensure quality care.

Appointment Reminders and Health-Related Communications

We may use your PHI to contact you regarding appointment reminders, test results, or information about treatment alternatives or other health-related benefits and services that may be of interest to you.

As Required or Permitted by Law

We may use or disclose your PHI when required or permitted by federal, state, or local law, including for public health activities, reporting abuse or neglect, health oversight activities, judicial and administrative proceedings, law enforcement purposes, to avert a serious threat to health or safety, for specialized government functions, and for workers' compensation.

Uses and Disclosures Requiring Your Written Authorization

We will obtain your written authorization before using or disclosing your PHI for purposes not described in this Notice. You may revoke your authorization in writing at any time, except to the extent that we have already taken action in reliance on it. We will not use or disclose your PHI for marketing purposes or sell your PHI without your written authorization.

Your Rights

You have the following rights regarding your PHI:

  • Right to Access: You may request to inspect and obtain a copy of your PHI maintained in our records.
  • Right to Amend: You may request an amendment to your PHI if you believe it is incorrect or incomplete.
  • Right to an Accounting of Disclosures: You may request a list of certain disclosures we have made of your PHI.
  • Right to Request Restrictions: You may request restrictions on certain uses and disclosures of your PHI. We are not required to agree to your request except in limited circumstances.
  • Right to Request Confidential Communications: You may request that we communicate with you in a certain way or at a certain location.
  • Right to a Paper Copy of This Notice: You may request a paper copy of this Notice at any time.
  • Right to File a Complaint: If you believe your privacy rights have been violated, you may file a complaint with our office or with the Secretary of the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.

Changes to This Notice

We reserve the right to change the terms of this Notice and to make the new provisions effective for all PHI we maintain. The current Notice will be posted on our website and available at our office.

Filing a Complaint

If you believe your privacy rights have been violated, you may file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights by visiting www.hhs.gov/ocr/privacy/hipaa/complaints or calling 1-877-696-6775.
