Telehealth Informed Consent

Pursuant to NRS 629.515 and NRS 200.620 · Effective Date: April 1, 2026

Introduction

Telehealth involves the delivery of health care services using interactive audio, video, or other electronic media between a patient and a health care provider who are not in the same physical location. This document provides important information about telehealth services offered by Dr. Rebecca Sinai, MD (“the Practice”).

By consenting to telehealth services, you acknowledge that you have read, understand, and agree to the following terms.

Nature of Telehealth Services

During a telehealth visit, your physician may review your medical history, conduct a focused examination (to the extent possible via video), discuss symptoms, recommend diagnostic tests, prescribe medications, and develop or modify treatment plans. Telehealth is not a replacement for in-person care when physical examination is medically necessary.

Benefits of Telehealth

  • Convenient access to care from your home or office
  • Reduced travel time and wait times
  • Timely evaluation of non-emergency concerns
  • Continuity of care while traveling
  • Follow-up visits without unnecessary office trips

Risks and Limitations

  • Technology limitations may affect the quality of the visit (e.g., connectivity issues, audio/video quality)
  • The inability to perform a hands-on physical examination may limit the physician's ability to diagnose or treat certain conditions
  • In rare cases, information transmitted may be insufficient to allow for appropriate medical decision-making
  • Delays in evaluation or treatment may occur due to technology failures
  • Security protocols are in place, but as with any technology, there is a potential risk of unauthorized access to your information

Your Rights

  • You have the right to withhold or withdraw consent to telehealth services at any time without affecting your right to future care or treatment.
  • You have the right to request an in-person visit in lieu of a telehealth visit.
  • You have the right to be informed of all persons present during the telehealth visit and to exclude anyone from the session.
  • All existing confidentiality protections under HIPAA and Nevada law apply to telehealth visits.
  • You have the right to access your medical records generated during telehealth visits.

Recording Disclosure (NRS 200.620)

IMPORTANT: Nevada is a two-party consent state.

Under Nevada Revised Statutes Section 200.620, it is unlawful to intercept or record a private conversation without the consent of all parties. Our telehealth visits are not recorded unless all participants provide explicit consent. If recording is requested for any purpose (e.g., for your personal records or for continuity of care), we will obtain your written consent before any recording begins.

Privacy and Security

All telehealth visits are conducted using HIPAA-compliant technology. We use reasonable safeguards to protect your health information during and after the visit. Your telehealth visit will be treated with the same confidentiality as an in-person visit.

Emergencies

Telehealth is not appropriate for medical emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately.

Cost and Insurance

Telehealth visits may be covered by your health insurance plan. You are responsible for any co-payments, deductibles, or out-of-pocket costs as determined by your insurance carrier. As a concierge patient, many telehealth visits are included in your membership.

Acknowledgment and Consent

By participating in a telehealth visit with Dr. Rebecca Sinai, DO, you confirm that you have read and understood this Informed Consent, that you have had the opportunity to ask questions, and that you voluntarily consent to participate in telehealth services.

Formal consent will be obtained through the Patient Fusion portal prior to your first telehealth appointment.
